REQUEST A MEDICAL CARD APPLICATION FORM
You may be entitled to a free eye exam and free glasses through your Medical Card. Fill in your name and address below and we will post you an application form. Fill in "Form LOA/1" and post to the HSE Optical Dept., Primary Community & Continuing Care, 25 Newcastle Road, Galway. The stamped form will be returned to your home address. Once you have received the form stamped and authorised by the HSE, you can fill in the online booking form to arrange your eye examination or if you prefer, you can call us on 091-386669. Please note that you are only entitled to an eye examination with your Medical card every two years, unless there is a necessity for a yearly eye examination. If you need a yearly eye examination, please contact us for further information.
